How can we support HCPs to raise patient safety issues?

  

There have been a number of incidents and subsequent reports over the past few years that have highlighted health care professionals raising concerns, however when we really get to the heart of the matter what does influence nurses in raising concerns?

The NMC Code Of Conduct clearly states that nurses should “escalate any concerns you may have about patient or public safety, or the level of care people are receiving in your workplace or any other healthcare setting and use the channels available to you in line with our guidance and your local working practices” and “Raise concerns immediately if you believe a person is vulnerable or at risk and needs extra support and protection” (NMC 2015) The NMC Code of Conduct applies to every registered nurse in the UK …. But does this really influence our decision to raise concerns? 

Similarly The GMC state “All doctors have a duty to raise concerns where they believe that patient safety or care is being compromised by the practice of colleagues or the systems, policies and procedures in the organisations in which they work. They must also encourage and support a culture in which staff can raise concerns openly and safely.”  

NHS Employers state re raising concerns “There has been much debate recognising that legislative drivers and guidance will only take us so far, and that taking action quickly and effectively is absolutely crucial – we are all well aware of the catastrophic effects when concerns are not raised and dealt with appropriately.” The link between culture and raising concerns if further strengthened in Sir Robert Francis’s report Freedom to Speak Up (2015) where he states “Raising concerns should be part of the normal routine business of any well-led NHS organisation” 

So what else is needed and what other influencers are there? What part does culture play in raising concerns? Noble (2015) makes the connection between culture and raising concerns “Protecting patients and staff from harm and ensuring dignity of care requires an open and transparent culture at all levels of the system, but getting it right is no easy task.” Mixed in with and hand in hand with culture we have values like the 6Cs thrown into the mix; does having a set of values such as care, compassion, communication, courage, competence and commitment help us to raise concerns? In developing a culture of compassionate care with underpinned with the 6Cs and visibly driven by healthcare professionals like Caremakers can this also be a factor in influencing our decisions to raise concerns? 

All of this leads us to the very interesting question “Culture or codes – which influences our decisions to raise concerns?” Do doctors, nurses and other HCPs raise concerns because their code of conduct states that they must or is it down to organizational culture? What helps nurses to raise concerns and how can we build on this? And how can we learn from this and how can we best support nurses to raise concerns? 

This #WeNurses discussion aims to explore the fundamental question of Culture or Codes - What can we do to support doctors and nurses to raise a concern if they witness a potential patient safety issue?

Front Line Healthcare Professionals Speaking Up for Patient Safety - Agents for Change and Faculty of Medical Leadership and Management Survey:

Recent inquiries from within NHS Trusts have revealed a culture where serious patient safety incidents were able to occur without being reported or adequately addressed. One of the concerns raised by these inquiries is that front line staff, including junior doctors, nurses and allied healthcare professionals, often do not feel empowered to speak up when they observe practices that compromise patient safety.

We would like to hear from junior doctors, nurses and other allied healthcare professionals about their experience of working on the front line and whether they are supported to speak up for patient safety.

The survey will be open until September 30th 2015.  Results from this survey will be shared and discussed with attendees at the Agents for Change conference on Saturday 24th October 2015 and we will publicise the results online soon afterwards. Results will also be shared with the NHS England Professional Standards team.

This survey is anonymous and confidential. We ask that you do not provide any details that might identify patients or colleagues

Sharon Lamont @lamont_sharon Sharon has been involved in research, management and leadership in the NHS for the past 15 years, and has been on a continuing journey of professional development and improvement. The starting point was as a nurse in Newcastle upon Tyne, followed by health care related research culminating in a PhD in community health, as well as several years teaching medical ethics to undergraduate medical students. She has worked on many national NHS programmes that have focussed on the redesign of clinical systems and improvements to the patient journey. Now a senior manager in the Medical Directorate of NHS England, Sharon is passionate about ensuring that good practice is supported by a clear and robust evidence base; that innovation at all levels is promoted and rewarded; that staff feel valued and cared for; and above all that patients always get the absolute best deal possible.

Raising concerns about patient safety is really not as simple as it first might seem there are many factors that influence our decision to raise concerns and although this #WeNurses discussion set out to explore whether it is culture or codes that influence our decisions to raise concerns the reality is much more complex.  The mind map below outlines all of the influencing factors to raising patient safety concerns that were shared by #WeNurses participants:
